Our Capital Advantage for Health Condition Management Programs are available to members with specific conditions who will benefit from long-term education and self-management support as well as monitoring of their health status.
Through our programs, individuals learn self-management skills to improve their health or the health of their children.
Disease Management Programs are available to members identified with specific diseases. Our programs currently manage the following conditions:
- Coronary Artery Disease
- Congestive Heart Failure
- Low Back Pain (HMO Only)
- Pediatric Asthma
- Pediatric Diabetes
The Capital Advantage for Health Condition Management programs are designed to deliver member education, facilitate timely, high-quality outcomes for its members, and promote appropriate use of healthcare resources. Program goals also include promoting overall health status of members, preventing exacerbation of symptoms and increasing member program participation and touch rates while providing a high degree of member program satisfaction through education and targeted interventions. In addition, care management staff focuses efforts on providing the latest evidence-based educational information to the member, while addressing the areas of medication compliance and symptom management.
The Capital Advantage for Health Pediatric Condition Management programs also offer Capital BlueCross network providers and practitioners the opportunity for enhanced communication with parents, members and other providers by providing notification of pertinent information related to changes in the member’s condition.
