Designed to promote coordination of care
Our Pre-admission Call Program (PAC Program) is designed to promote the coordination of care pre-operatively for selected elective inpatient surgical procedures and to provide pre-operative education to members preparing to undergo specific surgical procedures.
The PAC Program was developed to identify and address a patient's questions and concerns regarding hospitalization and provides information on the planned elective procedure. The program reviews pre-op services for selected inpatient surgical procedures and identifies potential discharge needs, and supplements and reinforces pre-op teaching provided by physicians, providing member education as appropriate.
The Capital BlueCross PAC program assesses a member's understanding of the anticipated length of stay, what to expect during hospitalization and answers questions about the scheduled procedure as well as expectations of hospitalization The pre-operative education provided through the program does not replace but rather supports and supplements the pre-operative teaching and preparation provided by the physician. It is implemented in coordination and collaboration with the patient's surgeon.
