I do not have health insurance. How do I apply for coverage?
If you know the product in which you would like to enroll, complete the Individual Accounts Subscriber Application for Enrollment. Do not send any money; you will be billed when your application is processed. If you need a copy of this form, please call 800.962.2242. If you do not know which coverage you'd like, please request our Individual Accounts packet that provides an overview, including rates, of the options we offer individuals and their families.
What is a Provider?
A provider is a hospital, health care facility, physician or other medical professional that provides health care services.
What is a Member?
A member is an individual who currently has health insurance coverage from Capital BlueCross.
What is a Broker?
A salesperson who has obtained a state license to sell and service contracts of multiple health plans or insurers, and who is ordinarily considered to be an agent of the buyer, not the health plan or insurer.
What is an Employer?
An Employer is a business or organization that contracts with Capital BlueCross to provide health insurance coverage for its employees.
What is a Primary Care Physician?
A duly licensed doctor of medicine or osteopathy who has a contract with Capital BlueCross. The PCP is the initial point of contact responsible for the member's health care. The PCP is also responsible for referring a member to a specialist within the network, when necessary.
