Network Care Providers
You must select a Primary Care Physician (PCP) for your child. Use our provider search to choose a PCP. You can also use this search to check if your child’s current physician is a KHPC participating PCP and you can include the name of the selected PCP on the application.

Check It Out®
If your child is enrolled in any of the low cost programs or the full cost program, you may sign up for Check It Out®. This program allows you to have your child’s monthly premium payment deducted from your checking account.
Health and Wellness
Capital BlueCross has programs to help your child stay healthy. Disease management programs help assess the health needs for members who have a chronic condition and provide education, counseling and related information designed to assist with the management of the disease or condition.
These disease management programs are available to CHIP members:
- Free ‘n Fit!℠ – Weight Management
Free 'n Fit is a free wellness program for CHIP children and their parents. The program provides resources for you and your child to live healthy. There are no classes to attend. Nutrition and fitness coaches work with you over the phone to help you and your child make simple changes that lead to good health. Email or call us at 877.330.2746 to learn more about Free 'n Fit.
- The Green Zone℠ – Managing Asthma
Managing your child's chronic breathing problem can be a challenge. The Green Zone program can help you manage medicines, breathing treatments and environmental hazards. You'll get tips to help your child feel better and enjoy being active. Email or call us at 800.543.7101 to enroll in The Green Zone.
- Kick It!℠ – Nicotine Cessation
Each day 6,000 youth try smoking for the first time. According to the National Center for Chronic Disease Prevention and Health Promotion, many young people start using tobacco by age 11 and many are addicted by age 14. We partner with the PA Department of Health to offer teens ages 14-18 Kick It!, a program to help them kick the tobacco habit. Find out more about Kit It! by emailing us or calling 800.634.4794.
- Smart Control 4Kids℠ – Managing Diabetes
Reinforcing healthy habits and helping your child manage his or her diabetes can be overwhelming. Smart Control 4Kids helps you and your child learn how to improve diet, exercise, medication management and attitude. You and your child work together to control diabetes and help prevent many future health complications. You'll get educational materials in the mail to support your child's wellness goals. Our free Nurse Line offers a trusted source for health information 24/7. You can speak with a registered nurse or access recorded health information.

Have your child’s Capital BlueCross insurance card handy when you contact Nurse Line. Call 800.452.2583 any time day or night. When prompted enter the PIN number 499. You can hear a recorded message about a health topic or talk to a nurse in person.
Your child’s privacy is important to us. Please read our Notice of Privacy Practices and the GLBA Notice. If you want Capital BlueCross to talk with another individual, not included on the CHIP application, regarding your child’s CHIP coverage, complete and return a Member Authorization Form.
Your child’s CHIP coverage must be renewed each year. We must receive the renewal form before the deadline on your letter or your child’s coverage will expire. You’ll get information about renewal 120 days prior to your child’s anniversary date.
If you prefer to mail renewal documents, please allow time for mailing and processing. We must cancel your child’s coverage on their anniversary date if we don't have a properly completed renewal form.
You'll need to provide income verification documents for all income received in the household. You may also have to provide tax filing information to verify your tax filing status and/or any deductions you claim on your taxes. Additionally, if your child is not a U.S. citizen you may be required to provide verification of their immigration status.
If there are any special circumstances regarding your income, please explain in detail on a separate sheet of paper.
