CPT 77332 Bundling in HDR Gynecologic Brachytherapy – Billing and preauthorization guidelines

KEY POINT: When doing HDR gynecologic brachytherapy using a vaginal cylinder or tandem and ovoid applicators, the treatment device work is already bundled within the insertion procedure.
For HDR gynecologic brachytherapy performed for gynecologic malignancies (including, but not limited to, endometrial cancer), the treatment device work is included in the applicable insertion procedure codes (CPT 57155 or 57156), consistent with established radiation oncology coding standards.
The insertion procedure codes (CPT 57155 or 57156) include all device-related work, such as:
- Selecting the device.
- Preparing the device.
- Insertion during treatment.
Because of this, CPT 77332 (treatment device):
- CPT 77332 remains on the prior authorization list, but when requested with these HDR procedures, the request will not be approved since it is not separately reimbursable.
- CPT 77332 should not be billed separately, as it is included in the insertion code.
- Separate billing may result in denial.
CPT 77332 may still be appropriate for authorization in other radiation therapy scenarios when it is not part of another service.
