Update Your Information

Use our provider finder to make sure we have your most current information about your practice.

You can submit changes through Availity Provider Portal. Complete the Provider Information Exchange forms on the Capital BlueCross Network Resource Center. Register or sign into update your information.

If you are a non-participating provider, call Customer Service at 866.688.2242.

Change requests may take up to 30 business days to complete.

Contact your Provider Relations consultant to help with these changes:

  • Mergers/acquisitions

  • TIN/EIN changes with a W9 entity

  • Adding new products or additional services

  • Existing providers establishing a new group
