PPO
Maximum freedom of choice for employees with our widest provider network.
PPO Tiered Plans
Plans with tiered provider networks can be offered at a lower cost to eligible employer groups. These plans address rising costs, while alleviating access concerns. Our tiered products are offered in select counties within Capital’s service area. They are designed to have a lower-cost in-network tier (called “Choice 1” for most plans and “Performance Plus” for Performance PPO), a higher-cost in-network tier (called “Choice 2” and “Performance Select” for Performance PPO), and an out-of-network tier.

- Advance PPO
- PPO Choice
- For Advance PPO, the network is the same as PPO Choice except Penn State Health and BlueCard claims are covered at the Choice 2 benefit level instead of Choice 1
- Employer groups located in Cumberland, Dauphin, Perry, and York counties.
- Capital Advantage PPO
- PPO Choice Select
- For Capital Advantage PPO, the network is the same as PPO Choice Select, except BlueCard claims are covered at the Choice 2 benefit level instead of Choice 1.
- Employer groups located in Adams, Franklin, Lancaster, Lebanon, Lehigh, Northampton, and York counties.
- Convenience PPO
- Performance PPO
- For Convenience PPO, the network is the same as Performance PPO except Penn State Health and BlueCard claims are covered at the Choice 2 benefit level instead of Choice 1.
HRA
Any medical plan with an employer-funded health reimbursement arrangement to help your employees pay for medical expenses. You choose how to administer the plan and how much to fund. HRA funds paid for employee claims are tax deductible for employers.
QHDHP
An IRS-Qualified High Deductible Health Plan, suitable for pairing with a tax-free Health Savings Account (HSA).

Designed for your Medicare eligible employees and retirees.
Capital Advantage EPO
An EPO plan for employers located in Cumberland, Dauphin, and Perry counties. Help manage patient care, reduce redundant or unnecessary treatment, and improve employee health — all with a plan that costs less than a traditional PPO.
Valley Advantage EPO
An EPO plan for employers located in Lehigh and Northampton counties. This carefully designed, integrated network of physicians ensures the comprehensive treatment of your employees while living and working in the Lehigh Valley and when traveling outside the service area through the BlueCard PPO network of providers.
COBRA
Healthcare continuation coverage to qualified employees and dependents who lose their insurance.
FSA
Flexible spending accounts allow your employees to set aside pre-tax dollars to pay for IRS-qualified out-of-pocket medical expenses. Dependent Care FSA is also available to help with daycare costs.
