There are multiple ways to apply for coverage. Let us help.

How do I complete the application?

  • Complete one application for all children living in the household that are under 19 years old whether you are applying for them or not
  • Answer all of the questions completely or it will delay processing your application
  • Include your acceptable proof of income and acceptable proof of citizenship and identity documentation
  • Include other documentation noted on the application
  • Include these individuals on your application:
    • yourself
    • your spouse or unmarried partner
    • anyone living in the household that are under 21 years old
    • anyone you include on your tax return, including those who don't live in the household

Apply online

COMPASS walks you through the steps to complete an application, including what documentation is needed. When you choose Capital Blue Cross in the Insurance Company Information section, if your child is found eligible for CHIP, your child's health care coverage will be provided by Capital Blue Cross.

If you need assistance completing your COMPASS application, call 800.543.7101 to request a COMPASS callback. A Capital Blue Cross Member Services Representative will help you schedule time with a CHIP Specialist to help you with completing your COMPASS application.

Other ways to apply

Families will be able to submit applications and verification documents in several ways:

  • Online on the COMPASS website
  • Upload photos of verification documents on the free myCOMPASS PA mobile app
  • Call COMPASS at 1.866.550.4355 or call the CHIP Call Center at 1.800.986.5437 and select option 2 to complete applications and renewals via phone
  • Mail, fax, or drop off items in person to your local CAO

Note: Families should not send any applications, renewals, or verification documents to their CHIP health insurance company or to the CHIP office in Harrisburg. This may delay the processing of their application.
