Give employees affordable health insurance and freedom of choice. On most plans, members see physicians and select hospitals of their choice with flat-rate copayments for office visits. PPO members have access to VirtualCare (for only a $5 copay on most plans) and wellness programs, including Healthy Blue Rewards and more.

Most Gold and Silver PPO plans include a choice of two prescription drug plans. Rx 0 does not require any deductible on prescription drugs -- only fixed copays. Rx 500 applies a $500 annual deductible to brand-name prescription drugs only.

The Bronze PPO includes a prescription drug plan with an integrated medical/Rx deductible.
